Information About Stolen Laptop and Potential Privacy Breach

On September 16, 2019 two individuals not affiliated with our company entered our premises without permission and stole a company laptop computer that was used by our nurse. We do not believe that any social security numbers, financial information, driver's license numbers, or insurance information was on the computer. We do believe that certain personal information about clients, including their name, date of birth, photo, and certain clinical information, such as diagnoses/conditions and our nurse's health notes, was contained on the computer. We have not received any information that the information was accessed or used by unauthorized individuals, but it is possible that client information on the laptop may have been compromised.

We have taken a number of steps to investigate this incident. We promptly reported the incident to the St. Paul Police Department and provided them with information to aid in their investigation, including surveillance video of our premises. We will continue to work with the police as they investigate this incident.

We know how important our client's personal information is to them. We regret this incident. We work hard to earn our client's trust and are committed to providing quality services. We have policies and procedures in place to protect the privacy of our clients. In response to this theft, we have taken even more actions to further improve the security of our operations.
